Maintenance request form and repair approval workflow

Report something that needs fixing, approve the cost when needed and confirm the repair.

A leaking pipe, an air conditioner that stopped, a machine making a new noise: somebody notices and tells whoever is nearby. After that it depends on memory. Urgent repairs wait behind small ones, contractors are hired without anyone checking the price, and nobody confirms the problem was actually solved.

This template gives every repair a request with an owner. The requester says where the problem is, picks the type (electrical, plumbing, air conditioning, machinery, building, furniture or other) and the urgency, from “can wait” to “urgent: safety risk or work stopped”, describes it and adds photos. Maintenance has one business day to diagnose it, choose between the internal team and an outside contractor, estimate the cost and schedule the job. Repairs estimated above $500 need an approval that records the cost center. The work is then done and recorded with a materials table and labor, the form calculates the final cost, and the requester confirms the problem is gone.

It works for offices, stores, warehouses, schools and small plants where a facilities person or a small maintenance team takes requests from many people. For example, a $180 faucet replacement goes straight to execution, while a $1,200 compressor repair waits for approval.

How it runs, step by step

The process reads from top to bottom. Each step goes to its owner with a deadline, and everything that happens is recorded in the request's timeline.

Make it yours

Take photos on the spot

Change Photos to a photo field, so pictures are taken in the moment and stamped with the date, time and location.

Plan preventive maintenance

Create a recurring schedule that starts a request automatically, for example an HVAC filter check every quarter.

Spread the work

Assign Execution to your technicians as a group with round robin or least busy assignment.

Open it to people without an account

Publish a public request form so tenants or visitors can report a problem and follow it on a tracking page.

Frequently asked questions

What should a maintenance request form include?

The exact location, the type of problem, how urgent it is, a short description and photos. Those five answers let maintenance triage the request without walking over to look first.

What is the difference between corrective and preventive maintenance?

Corrective maintenance fixes something that has already failed, which is what most maintenance requests are. Preventive maintenance is planned in advance, such as filter changes or inspections, to keep failures from happening. Both work better when every job is recorded with its cost.

Who should approve repair costs?

Small repairs should go ahead without approval so they do not wait. Above an amount you set, a facilities manager or budget owner should approve before work starts, especially when an outside contractor is involved.

How should maintenance requests be prioritized?

Safety risks and work stoppages first, then problems that affect many people, then everything else. Ask the requester for the urgency on the form, and let maintenance confirm it after the diagnosis.
