When someone leaves, the risks are quiet ones. An email account stays active for months, a laptop never comes back, a client thread has no owner, or the final paycheck goes out late. None of it looks urgent until it is.
This template runs a departure in four steps. The departure notice records who is leaving, their role, the last working day, the reason and how the notice period is handled. The manager then has five business days to hand over ongoing work, transfer files, tell the team and name who takes over. IT has one business day to block and forward the email, revoke system access, release licenses, collect badge and keys, and record whether the equipment came back; if anything is missing, the form asks what. HR finishes with a closing checklist that covers final pay, signed termination documents and benefits, plus the termination date, the documents themselves and notes from the exit interview.
It works for any company where a departure touches HR, IT and a manager, whether the person resigns, is let go or reaches the end of a contract. Everything done along the way stays in the request’s timeline, so you can show later what happened and when.
