Most purchases start as a message: “Can we buy two monitors for the new hires?” Someone says yes, someone else orders, and when the invoice lands nobody remembers who approved it, what it was for, or whether the monitors ever arrived. A purchase request form fixes the first part of that problem by asking the same questions every time.
This template takes a request from “I need this” to “it arrived”. The requester lists the items with quantity and unit price, the form adds up the estimated total, and a needed-by date and any quotes can be added. If the total is over $1,000, a director approves it and picks the budget line; smaller purchases go straight to whoever places the order. That person records the supplier, order number, final amount and expected delivery, and the request returns to the requester to confirm that everything was received.
It is built for office managers, operations leads and finance teams who buy often enough to lose track, but not so often that they need a procurement system, and who want one record per purchase instead of an email thread.
