Performance reviews fail less from bad forms than from poor follow-through. Self-assessments come back late, managers fill in scores the night before, the feedback meeting never makes it onto the calendar, and the development plan agreed in the room is forgotten by next quarter.
This template runs one review per employee in five steps. The employee starts with a self-assessment for the period (first half, second half, full year or probation): main achievements, difficulties, a score from 1 to 5 and what they want to develop. The manager then scores quality of work, deadlines and commitment, teamwork, and initiative and autonomy from 1 to 5, the form calculates the average, and the manager writes strengths and points to improve. Next comes a feedback meeting, recorded with its date, a summary and a development plan listing actions, support needed and deadlines. The employee acknowledges the feedback and can add comments. If the average is below 3, HR adds a follow-up plan and sets the next review date.
It suits companies that want a simple, repeatable review cycle with a written record, without adopting a dedicated performance management suite.
